I understand that my participation in a Worthwhile Mile event is a potentially hazardous activity, which may result in serious bodily injury, including permanent disability, paralysis, and death. I fully assume all risks associated with my participation, and discharge The Worthwhile Mile project and its organizers, employees, directors, agents, volunteers, officers, and partners from all actions, claims, or demands for damages arising from my participation in a Worthwhile Mile event. The foregoing release shall be binding upon me personally as well as upon my heirs, executors/administrators, and family members.

I agree to wear a helmet and safety triangle at all times while cycling before and during a Worthwhile Mile group ride event. I understand that Worthwhile Mile Trip Leaders will evaluate my competence with respect to cycling, traffic laws and rights of the cyclist, hand signaling and effective communication with other drivers, cyclists, and pedestrians sharing the road. I understand that trip leaders reserve the right to dismiss me from participation in the Worthwhile Mile group ride event if, at any point before or during the ride, trip leaders conclude I am do not exhibit the before stated competencies.

What is your skill level on the bike?
(Your answer will not affect your eligibility to participate in the wm4WSEF RIDE, we'd simply like to get an idea of your experience.)
1 - BEGINNER (You plan on learning to ride a bike just to go on this trip OR you could count the number of times you've been on your bike in the last five years on one hand.)
2 - CASUAL CYCLIST (You may ride around town, campus or on the bike trail for fun. 10 or 15 miles sounds like a great time!)
3 - KNOWLEDGEABLE CYCLIST (You're no stranger to riding in unfavorable weather. You've enjoyed wearing spandex for 20 or 30 mile rides.)
4 - ADVANCED CYCLIST (You may commute most every day, and you can't wait to rack up some 65 mile rides as soon as spring hits. You just love jerseys and ride patches!)
5 - EXTREME CYCLIST. You have a HRM. You race on a cycling team. You've toured across the country self-supported. You may have been sponsored and you know you're just this awesome.
